Evans Counseling and Consulting is committed to providing you the best possible experience. Payment is due and processed the morning of the appointment. Accepted methods of payment include debit card and credit card. Please note a  credit card is required to be on file. If payment is not obtained or is declined, staff will make an attempt to contact you and collect payment before your appointment. If payment still cannot be collected or contact is unsuccessful, the appointment will be canceled and a fee will be assessed.

After 3 occurrences of a declined payment during your course of treatment in a year, an inconvenience fee of $20 will be added to your account for each incident or pre-payment may be required at the time of  scheduling.

Fees for administrative services will be billed, if requested, at half of the individual therapeutic rate for a minimum of 60 minutes. This includes, but is not limited to treatment summary requests, professional letters, work forms, and phone/conference calls.

Evans Counseling & Consulting does accept insurance, however, not every service can be paid by insurance (Couples/Marriage Counseling and Immigration Evaluations are self-pay only). We accept the following insurances:

  • UnitedHealthcare (UMR, GEHA)   
  • BlueCross BlueShield (Regular, State, Federal, PAI)                  
  • Medicare
  • Aetna
  • Cigna

Under the No Surprises Act, Good Faith Estimates are provided. We will verify your insurance benefits and provide an approximate cost for services as determined by your health insurance plan. However, please keep in mind that it is an estimate and the processed claim may override what the representative communicates to us.

 

Cancellation Policy

If you are unable to keep your appointment, please provide a 24-hour notice to avoid the cancellation and no-show fee. Failure to provide a 24-hour notice prior to your scheduled appointment time will result in a $50 late cancellation fee. A $100 no-show fee will be charged if you fail to show for your scheduled appointment without notice. Clients are given an annual “free pass” that can be used to waive one late cancellation fee. If you late cancel, but reschedule within the same week, no fee will apply.

Clients who have not scheduled an appointment in 4 weeks since their last appointment will be considered terminated and availability for resuming services will be determined by the counselor’s current caseload.

Clients who frequently cancel or reschedule their appointments may be terminated at the counselor’s discretion. It will be assumed that you are not ready for therapy or to continue forward in the therapeutic process.
