Upon booking your appointment, you will immediately receive the intake paperwork to the email address you provided. 

For individuals: Please note that the new client intake paperwork must be completed 3 days before your booked appointment or the appointment will be canceled. This will allow us time to verify your insurance benefits and update you on your financial responsibility, if any. Credit card information is required to secure your appointment date and time, however, you will not be charged until the morning of your scheduled appointment. 

For couples: Please note that the new couples intake paperwork must be completed by both parties 3 days before your booked appointment or the appointment will be canceled. Insurance is not accepted for this service. At booking, you will be charged a $100 deposit that will go towards the $160 fee of your scheduled appointment. The remaining balance will be charged the morning of your appointment.
