Upon booking your appointment, you will immediately receive the intake paperwork to the email address you provided. 

For individuals: Please note that the new client intake paperwork must be completed 3 days before your booked appointment or the appointment will be canceled. This will allow us time to verify your insurance benefits and update you on your financial responsibility, if any. Credit card information is required to secure your appointment date and time, however, you will not be charged until the morning of your scheduled appointment. 

For couples: Please note that the new couples intake paperwork must be completed by both parties 3 days before your booked appointment or the appointment will be canceled. Credit card information is required to secure your appointment date and time and will be charged a $25 booking fee that will go towards the overall couples counseling fee. We will verify both parties insurance benefits and update you on your financial responsibility for your couples appointment.
